North Star Manor Assisted Living

North Star Manor’s Assisted Living community provides a comfortable, safe option that still feels like home.

Our Assisted Living building is physically connected to North Valley Health Center, one of Northwest Minnesota’s most progressive hospitals. For residents, a sense of community is always present, a little fun is part of every day and visitors are always welcome.

North Star Assisted Living includes 20 apartments.

BRONZE

All base services plus: Initial and periodic Registered Nurse Assessment 24-hour qualified staff assistance with arranging/scheduling appointments and/or transportation Obtain monthly weight and vital signs Medication management (obtaining physician orders, processing physician orders, ordering meds) Medication administration or Medication reminders up to 2 times per day Meal setup (opening containers, cutting food etc.) Bath or shower 1 per week (up to 20 minutes) Personal care – help with dressing/grooming 1 time per day (up to 15 minutes) Wellness check 1 time per day

SILVER

All base services plus: Initial and periodic Registered Nurse Assessment 24-hour qualified staff assistance with arranging/scheduling appointments and/or transportation Obtain monthly weight and vital signs Medication management (obtaining physician orders, processing physician orders, ordering medications) Medication administration Bath or shower 2 times per week (up to 45 minutes) Personal care- help with dressing/grooming up to 2 times per day (up to 30 minutes) Daily bedmaking Daily incontinence assistance up to 2 times per day (up to 30 minutes) Wellness check 2 times per day

GOLD

All base services plus: Initial and periodic Registered Nurse Assessment 24-hour qualified staff assistance with arranging/scheduling appointments and/or transportation Obtain monthly weight and vital signs Medication management (obtaining physician orders, processing physician orders, ordering medications) Medication administration Bath or shower 3 times per week (up to 60 minutes) Personal care assistance, such as help with dressing and grooming, up to 3 times per day (up to 60 minutes) Daily bedmaking Daily incontinence assistance up to 3 times per day (up to 45 minutes) Wellness check 3 times per day Escort to meals or in-house activities/programs Additional delegated nursing services up to 45 minutes per day

Delegated nursing services include, but are not limited to:

Nebulizer treatment
Blood glucose monitoring
Simple dressing changes
Pacemaker checks
Supportive stocking
